Iran already had shortages of electricity and water before the war, thanks to clerical rule. Teheran averages 2 hours a day of electricity outages.
The infrastructure that could have been destroyed but wasn't is Iran's 5 oil refineries, which provide 80% of domestic needs.
If I had read about this operation in a spy novel, complete with secret Mossad drone bases in the outskirts of Teheran, I would have dismissed it as far-fetched.
